Where Strongholds appear depends on the version of the game you're playing: in Bedrock Edition, they are random, but are usually found underneath Villages. In the Java Edition, they can appear within one of eight rings that are centered at the world spawn point: there can only be 128 Strongholds total, and beyond 24,320 blocks, they stop appearing.

The only way to locate a Stronghold (outside of pure chance) is to use an Eye of Ender. Here's how to do this:

Eye of Enders are crafted using an Ender Pearl and Blaze Powder. Ender Pearls have a 50% chance to be dropped by a defeated Endermen, while Blaze Powder can be crafted from Blaze Rods, which are dropped by Blazes found in The Nether's Nether Fortresses.
While holding an Eye of Ender, Use it and it will fly in the direction of the nearest Stronghold. However, it has a 20% chance of shattering after it's used, and thus lost for good. Ergo, it's best to have several Eye of Enders, especially if you intend on activating the End Portal.

While Strongholds can have many different layouts, there are three particular rooms that deserve highlighting:

End Portal Room: Probably the reason you're even looking for a Stronghold, End Portal Rooms are the only places in the game where you can travel to The End, by placing an Eye of Ender in each of the empty sockets (each socket has a 10% chance of already having one). It is not a guarantee that a Stronghold will have an End Portal Room, but they are more common in the Bedrock Edition.
Library: Strongholds can have between 0-2 Libraries, and can be either single or double-storeyed. The abundance of bookshelves make them a great source of gathering Books.
Storeroom: A double-storeyed room that contains a single Chest inside.
Altar Corridor: A corridor with an altar in the middle with a single Chest on top. There can be between 0 - 4 of these corridors per Stronghold.

Stronghold Chests will have different potential items inside depending on what room they are found in.

Steps to Find a Stronghold

1. Throw the Eye of Ender

First, you need to select an Eye of Ender in your hotbar and then throw it into the air to see which direction it goes.

The game control to throw the Eye of Ender depends on the version of Minecraft:

For Java Edition (PC/Mac), right click to throw the Eye of Ender.
For Pocket Edition (PE), tap where you want to throw the Eye of Ender.
For PS3 and PS4, press the L2 button on the PS controller.
For Xbox 360 and Xbox One, press the LT button on the Xbox controller.
For Windows 10 Edition, right click to throw the Eye of Ender.
For Wii U, press the ZL button on the gamepad.
For Nintendo Switch, press the ZL button on the controller.
For Education Edition, right click to throw the Eye of Ender.

You will see the Eye of Ender float up into the air, hover, and then drop to the ground. This is the direction that you need to go. If the Eye of Ender did not break, you can pick it up and throw it again.

TIP: If you are in Creative mode, it is easiest to fly up in the air to get a better look at where the Eye of Ender is leading you.

Continue to throw the Eye of Ender into the air until it hovers over one spot. This is the location of your Stronghold.
